• I understand that Vasectomy Central or Dr. Filion will never ask for payment or request that I click on any links. (Beware of scammers!)
• I understand that this prize is only valid for Game 3 or 4, and I confirm that the nominee is available to attend that evening.
• I understand that if I become unavailable, I will let Dr. Filion know as soon as possible so he can select a new winner.
• I understand that this prize is non-transferable and cannot be exchanged for cash.
• I understand that this prize is intended for a father-and-child duo, not two adults.
• I understand that the winner will be called directly by Dr. Filion using the number submitted in this form.
• I understand that the winner will have 12 hours to confirm acceptance of the prize. Failure to do so within the allotted time will result in the selection of a new winner.
• I understand and confirm that the nominee:
  • Is the father or legal guardian of the child attending, and
  • Resides in Alberta.
• I understand that this contest is subject to change or cancellation at any time.
• I understand that by entering, participants agree to allow Vasectomy Central to use the winner's name for content or promotional purposes.
